Tattoo Pain Chart: Which Spots Hurt the Most (And Least)?

Pain is the first question almost every tattoo client asks. The honest answer: yes, it hurts — but the amount varies dramatically by placement, your personal pain tolerance, and the type of tattooing involved. At Born Again Tattoo in Columbia, SC, we prep every client honestly so there are no surprises in the chair.

Here’s a comprehensive tattoo pain guide organized by body area.

Low Pain Areas

Outer Upper Arm / Bicep

One of the most comfortable spots. Lots of muscle and fat, minimal bone proximity. Great starting point for first tattoos and sleeve beginnings.

Outer Thigh

The outer thigh is one of the most comfortable placements on the entire body. High muscle and fat content, distance from bone. Excellent for larger pieces.

Calf

Generally comfortable to tattoo, especially the outer calf. Less fleshy than the thigh but still manageable. Good for medium-to-large standalone pieces.

Outer Forearm

The outer forearm has decent muscle coverage and handles tattooing relatively well. A popular and approachable placement.

Shoulder / Upper Back

The shoulder and upper back area offer good muscle coverage. Comfortable for most clients, with the exception of areas directly on the shoulder blade bone.

Medium Pain Areas

Inner Forearm

Slightly more sensitive than the outer forearm due to thinner skin and more nerve endings. Still very manageable for most people.

Inner Bicep

The inner arm has more nerve endings and thinner skin than the outer bicep. Moderate pain level — more tolerable than many people fear.

Lower Back

The mid-to-lower back (away from the spine) is moderately painful. Spine proximity increases sensitivity significantly.

Chest (Center/Lower)

The chest can range from moderate to painful depending on how close you get to the sternum and collarbone. The lower, fleshier parts of the chest are more manageable.

Wrist

The inner wrist is more sensitive than many expect due to thin skin and proximity to tendons. Still very tolerable for most — just be prepared.

High Pain Areas

Ribs

The ribs are widely considered one of the most painful spots for tattooing. Very thin skin directly over bone, combined with the movement of breathing during the session. Not recommended for your first tattoo. But the results are often stunning.

Spine

Tattooing directly on the spine is intense. The vibration travels through the bones and the sensation is distinctly unpleasant. Highly recommended to break spine work into manageable sessions.

Sternum / Chest Plate

The breastbone is thin-skinned and bony — a difficult tattooing surface that is genuinely painful. Worth it for the right design, but plan accordingly.

Hands and Fingers

Hands are full of nerves and bone. They also fade fastest due to constant use. Painful to tattoo and require more maintenance over time.

Feet and Ankles

Thin skin over small bones with lots of nerve endings. Feet are among the most painful spots for most clients. The results can be beautiful — just know what you’re signing up for.

Neck

The neck — especially the front and sides — is very sensitive. Behind the ear and the nape are more manageable than the throat or sides.

Head / Scalp

Shaved-head tattooing has grown in popularity. The scalp is loud (the needle vibration resonates through the skull) but the pain itself is often described as less intense than people expect. Still: this is advanced territory.

Armpits

One of the most universally agreed-upon painful placements. Dense nerve clusters make this extremely uncomfortable. Rare for mainstream tattoos for good reason.
